"HOW TO" GUIDELINES FOR INDUSTRIAL MENTORS

This is an academic program therefore please comply with these guidelines.

1. Register your company by completing the Company Registration Form and create a user name and password (the same user name and password can be used by all mentors within the same company).
2. You will be notified by the Internship Coordinator ahunter@ucsd.edu when your registration is processed. You may then screen the intern applications on-line by using your username and password to log onto the BIO website.
3. Contact and arrange an appointment to interview any of the registered students in whom you are interested. (You may want to screen the student via phone, prior to the interview.)
4. If you wish to accept the student as an intern:
1. Notify the Internship Coordinator, April Hunter ahunter@ucsd.edu or 858-534-0913.
2. Verify internship start date.
3. Log on at website and complete the "Learning Agreement Form" (View Sample Form) (Complete Form).
5. At the end of each internship quarter, evaluate and submit the student's performance evaluation (View Sample Form) (Complete Form)
